To apply, please:

• download and complete the Queensland Government DA Form 1 and Application for site specific environmental authority (PDF 84 KB)
• and return it to us with full payment. You can submit your application in person at our customer service centres, by post or email. Please see connect with us for our contact information
• include a plan with your application.

Delays in the assessment and approval process will occur if you don’t include all of the required information.

Licence amendments

To make an amendment to the existin Environmentally relevant licence, download and complet the Application for amendment of environmental authority (PDF 92 KB) and return to us.

Licence transfers

To transfer all or part of the existing environmentally relevant licence, download and complete the Application to transfer all or part of an environmental authority (PDF 91 KB) and return to us.

Surrendering a licence

To surrender an existing environmentally relevant activity licence, download and complet the  Application for surrender of an environmental authority (PDF 73 KB) and return to us.

Amalgamation

To amalgamate two or more environmental authorities into a new authority, download and complete the Application to amalgamate environmental authorities (PDF 74 KB) and return to us.

Transitional environmental program

To apply for a transitional environmental program, please download and complete the Transitional environmental program submission (PDF 60 KB) and return to us.
