Program Delivery

The Active & Healthy program for 2025/2026 is broken into 4 blocks:

  • Block 1 - 1 July to 30 September 2025 
  • Block 2 - 1 October to 14 December 2025 
  • Block 3 - 12 January to 31 March 2026 
  • Block 4 - 1 April to 30 June 2026

Service providers can choose whether to deliver:  

  • weekly activities 
  • one-off activities 
  • courses.

Providers can choose whether to deliver all year around or during school terms only. No activities are delivered on public holidays. The Active & Healthy program breaks for 4 weeks over December/January.  

Participant Registration 

  • Participants arrive for their first class and complete a registration form. Participants are required to complete a new registration form each block. 
  • Registration forms must be completed and signed by participants before starting the activity.  

Registration forms may be printed and completed by the participant manually, or the Active & Healthy team can assist providers to incorporate the required information into their existing online forms or web bookings (requires final approval from the Active & Healthy team).

The completed registration forms enable providers to fulfil their reporting requirements. 

Provider Reporting

  1. At the end of each block, you’ll complete an online provider report for each funded activity. The link for reporting will be provided to you by the Active & Healthy team. Reports are required to be submitted by the due date. 
  2. The online provider report will ask questions about your activity and you’ll be required to upload any registration forms completed during the block, and an invoice for payment.  
  3. Once reporting is complete, the invoice will be paid as per the amount outlined in your contract. Invoices will be paid within 14 days. 

Questions that may be asked as part of the service provider reporting include: 

  • Organisation name 
  • Name of activity 
  • Day/time of activity 
  • How many different participants attended during the block? 
  • How many participants attended the activity for the first time? 
  • Did you have to turn away any participants? 
  • How many males/females attended? 
  • What Countries were participants born in? 
  • How did participants hear about the program? 
  • Was the location suitable for the activity? 
  • Were there any barriers for participants to attend?
